The IMPACT program is designed to provide treatment in a team setting, offering a wide array of clinical, medical, or rehabilitative services during face-to-face interactions, which are designed to help individuals to live independently in the community, or to assist in the transition to independent living from a more restrictive setting.

Accessibility IMPACT provides 24-hour, seven-days a week emergency response capability including psychiatric services. Face-to-face contacts with the enrolled consumer occur as frequent as are needed on an individual basis. Services are provided in locations other than the team office, such as, the consumer’s home, community settings, etc.
The IMPACT offices are centrally located and easily accessible through the public transportation services. The offices are handicap accessible at ground level, or through the use of ramps and handrails. Should the need arise for bilingual or sign language translation; we first try to get assistance from the consumers family or support system. If the client has limited supports, we will contact community translators, or contract with a translator to assist us to provide face-to-face services. We also contract with a phone translation service called "Language Line".
The Professional Team Each team has a Physician, an RN, a Master’s Level Clinical Team Leader and a Bachelor Level Team Coordinator. The Community Advocates working on the teams may have a high school diploma up to a master’s degree. This interdisciplinary approach to the team make up provides a vast array of experience to serve those most in need. Additionally, individual staff are trained in many different areas of expertise to include: substance abuse, supported employment, forensic services and Dialectical Behavioral Therapy.

ACT I, II, and III
|
|
|
- Consumer is an adult (age 18 or older)
- Consumer has Severe & Persistent Mental Illness (SPMI) or a Severe Mental Illness (SMI) with a major affective disorder, psychotic disorder and/or personality disorder (may have a dual diagnosis)
- Consumer has experienced two hospitalizations, two crisis home placements, two episodes of on-site crisis stabilization, or a combination of any two listed within the past one year period, or
- Consumer has had repeated crisis intervention services of other types within the last six month period
- Consumer has a Level of Care score of 3 or higher
- Consumer has been treated with less intensive services without success
- Natural/Community resources are inadequate to meet the consumer’s needs
- Consumer resides in Jackson or Hillsdale Counties

ACT IV
|
|
|
- Consumer is an adult (age 18 or older)
- Consumer has Severe & Persistent Mental Illness (SPMI) or a Severe Mental Illness (SMI) with a major affective disorder, psychotic disorder and/or personality disorder (may have a dual diagnosis)
- Consumer is in jail or at risk of going to jail as a result of their mental illness, or
- Consumer is currently on probation, or
- Consumer is currently on parole or will be released soon from prison
- Consumer has a Level of Care score of 3 or higher
- Natural/Community resources are inadequate to meet the consumer’s needs

Exclusion Criteria
|
|
|
- Consumer is under the age of 18
- Consumer has no MI diagnosis
- Consumer is receiving psychiatric services from another provider
- Consumer is receiving Community Living Training, Supports and Skill Building assistance from another provide
