Medication Delivery

Medication delivery is offered to consumers who have chronic difficulties taking their medications as prescribed which could result in hospital or crisis home admissions, or may prevent the need for more restrictive services. Medication Delivery Services are provided by specialty trained staff under the supervision of an RN.

Admission Criteria:

• Individual is 18 or older
• Individual resides in Jackson or Hillsdale County
• Individual has a Severe & Persistent Mental Illness, a Severe Mental Illness, a Co-Occurring Disorder
• Individual is receiving Network authorized Psychiatric and Case Management Services

The intent of the program is to assist consumers in identifying and implementing strategies to take their behavioral health medications as prescribed with the goal of moving the person from daily medication drops to taking their medication independently. A major component of the program is medication education and adherence monitoring.
